Box Breathing: How to Do It, and What the Evidence Shows

Slow breathing with the four phases held equal. What the trials show, what they do not, and when a longer exhale is the better tool.

What Box Breathing Is

Box breathing divides one breath into four equal parts: inhale, hold with the lungs full, exhale, hold with the lungs empty. Draw those four phases as sides of equal length and you get a square, which is where the name comes from. It also travels as tactical breathing or combat breathing, having been adopted into military and law-enforcement stress training, and that association is a large part of why it is the breathing technique most people can name.

The standard count is four seconds a side. That works out at sixteen seconds per breath, or 3.75 breaths a minute — against a normal resting rate somewhere between 12 and 20. Slowing your breathing by a factor of four is the whole intervention. Everything else is scaffolding to help you do it without counting wrong.

Two things follow from that, and they matter for the rest of this page. The first is that box breathing is a member of a family — slow-paced breathing — and most of the research you will see cited for it was done on the family, not on the square. The second is that the equal-hold structure is the one thing that genuinely distinguishes it, and the holds are not neutral. They are what make it harder than most alternatives, and they are the reason it is unsuitable for some people entirely.

How to Do Box Breathing

  1. Sit upright and exhale fully. Empty the lungs before you start, or the first inhale lands on top of air you never let go of. Feet flat, hands loose, jaw unclenched.
  2. Inhale through the nose for four. Aim for the belly rising before the chest. Comfortably full, not maximally full — a strained inhale is the most common reason people give this up.
  3. Hold for four. Hold with the airway open and the throat relaxed, not by clamping shut and bearing down. If you feel pressure building in your head, you are pushing rather than holding.
  4. Exhale through the nose or mouth for four. Steady and controlled, the same speed the whole way out. Do not dump the air in the first second and then wait.
  5. Hold empty for four. This is the phase people find hardest and the one they drop first. If it makes you anxious, drop it deliberately rather than rushing it — see the variations below.
  6. Repeat for four to six rounds to start. Six rounds is about a minute and a half. Stop earlier if you feel lightheaded, tingly, or more agitated rather than less.

Counting in your head works, but it competes for exactly the attention you are trying to settle, and most people drift faster than they think — the count creeps shorter as you relax. The timer in the sidebar paces it for you, and the full timer adds sound cues and a dimmed screen.

What the Evidence Actually Shows

Box breathing is well studied as a member of the slow-breathing family and thinly studied on its own. The largest pooled estimate available is a meta-analysis of breathwork trials generally, which found small-to-medium benefits for stress, anxiety and depression while cautioning against “a miscalibration between hype and evidence” in the field.2 Below, each claim is rated against research that tested that claim, not against research on breathing in general.

The authors concluded tactical breathing suits passive coping, and that something else is preferable when a task has to be done well.3

That last row is worth sitting with, because it is the claim box breathing is almost always sold on. The Navy SEAL framing implies performance under pressure, and the one study that put tactical breathing against a simpler alternative on a task requiring active concentration found the alternative produced better performance — while the tactical pattern was better at bringing arousal down.3 Those are not the same goal, and the popular version of the claim collapses them.

Why It Works, and What That Does Not Prove

The mechanism is not mysterious. Breathing slowly, particularly with a long controlled exhale, increases the influence of the vagus nerve on the heart. That shows up as higher heart rate variability and more pronounced respiratory sinus arrhythmia — your heart rate rising slightly on the inhale and falling on the exhale. Systematic review of slow breathing finds those autonomic shifts fairly consistently, along with EEG changes and subjective reports of calm and alertness.4

Mechanism Is Not Outcome

That slow breathing raises heart rate variability is well measured. That a given rise in heart rate variability makes you calmer, sleep better, or perform better is a separate claim needing separate evidence. Most breathing content collapses the two, which is how a plausible mechanism gets sold as a proven result. The review cited above makes the point itself: none of its included studies directly measured the correlation between the physiological changes and the psychological ones.4

There is a second wrinkle specific to the square. Much of the slow-breathing literature converges on roughly six breaths a minute — near the resonance frequency where the baroreflex produces the largest heart-rate oscillations. Box breathing at four seconds a side runs at 3.75 breaths a minute, which is slower than that. Slower is not automatically better, and there is no direct evidence that the square shape beats an even count at six breaths a minute for calming.

When Something Else Is the Better Choice

Box breathing is a good default and a poor universal. Three situations where the research points elsewhere:

  • You want the fastest possible drop in arousal. Cyclic sighing — a double inhale followed by a long exhale — beat box breathing on mood and on respiratory rate in the one trial that compared them directly.1 It also has no holds, so it is easier to do badly and still get the effect.
  • You have to perform a task well immediately afterward. Prolonged exhalation produced better task performance than tactical breathing under stress, even though tactical breathing calmed the body more.3
  • The holds make you anxious. For some people a breath hold is itself a threat cue, and the empty hold in particular can increase air hunger rather than settle it. That is a reason to change technique, not to push through.

Where box breathing genuinely earns its place is structure. It is symmetrical, so it is easy to remember and easy to pace without a device, and the equal counts give a wandering mind something to hold onto. For a lot of people that is worth more than a marginally better protocol they will not remember at the moment they need it.

Notice that the last two rows are no longer box breathing. That is fine. The goal is a slower breath with a controlled exhale, and the square is one route to it rather than the destination.

Safety and Who Should Skip the Holds

Before You Practice Breath Holds

Never practice breath holds in or near water, and not while driving. Talk to a doctor first if you are pregnant, or if you have a cardiovascular condition, uncontrolled blood pressure, epilepsy or a seizure history, glaucoma, a respiratory condition such as asthma or COPD, or panic disorder. If you feel faint, get chest pain, or become short of breath, stop.

Lightheadedness, tingling in the hands or around the mouth, or rising panic are signals to stop, not to push through. They usually mean the inhale is too big or the holds too long. Drop to a shorter count, or drop the holds entirely with 4-0-6-0.

Building the Habit

The trial that tested box breathing over 28 days used five minutes a day.1 That is a reasonable target, and it is worth noting what it implies: the mood benefit measured there accumulated over weeks of daily practice, not from a single session. A single session is for the acute effect — settling down before a difficult conversation, after a near-miss in traffic, in the ten minutes before sleep. Both uses are legitimate, but they are different, and evidence for one is not evidence for the other.

Practically: pick a fixed anchor rather than a time of day. After you sit down at your desk, before you start the car, when you get into bed. Five minutes is roughly 19 rounds at four seconds a side, which sounds like a lot and takes less time than reading this section.

Common Questions


How long before box breathing does anything?

The acute effect — a slower heart rate and a subjective drop in tension — usually arrives inside two to three minutes, which is four to six rounds at the standard count. The mood benefit measured in the 28-day trial was a different thing entirely, accumulating over weeks of daily practice.


Is box breathing better than 4-7-8?

For heart rate variability, neither: a 2025 study of 84 students compared square breathing, 4-7-8 and two six-breaths-a-minute patterns, and plain six a minute beat both.5 For calming, the point goes to 4-7-8 on reasoning rather than a direct test — its exhale is twice its inhale, and exhale-weighted patterns did better than box breathing in the one trial that compared several techniques. Box breathing’s advantage is the count: symmetrical is easier to remember at the moment you need it.


Can I use box breathing during a panic attack?

Breath holds can make panic worse for some people, because the sensation of air hunger is itself one of the things panic feeds on. If you want a breathing tool for acute panic, an exhale-focused pattern with no holds is the safer starting point. Anyone getting recurrent panic attacks should be talking to a clinician about it rather than managing alone.


Why does the empty hold feel so much harder than the full one?

Because the urge to breathe is driven mostly by rising carbon dioxide rather than falling oxygen, and it climbs faster from an empty chest. The empty hold is the phase most people shorten without noticing. Dropping it deliberately with a 4-4-6-2 count is more useful than doing it badly.


Do the Navy SEALs actually use this?

Box-style breathing is genuinely taught in military and law-enforcement stress training under names like tactical or combat breathing. That says something about how memorable and teachable the pattern is. It is not evidence of effectiveness — an institution adopting a technique is not the same as a trial testing it, and the one study of tactical breathing under load found a simpler pattern produced better task performance.


Does it have to be four seconds?

No. Four is a convention, not a finding. What matters is that the breath is slow, the exhale is controlled, and the count is comfortable enough that you keep doing it. If four seconds leaves you straining, use three.


The Short Answer

Box breathing works, in the sense that slow paced breathing reliably reduces self-reported stress and shifts autonomic balance in the direction you would want. It is easy to learn, easy to remember, and free. What it is not is the best breathing technique available, and it is not a focus aid — those two claims carry most of its popularity and neither survives contact with the research that tested them. Use it because the structure suits you. If you want the fastest drop in arousal, use something with a longer exhale and no holds.
