What This Timer Is Doing
One variable, isolated
Four in, eight out. Twice as long out as in, five breaths a minute, nothing held. It takes the single element most consistently linked to parasympathetic activation — a long exhale — and strips everything else away.
The physiology is not in dispute. Vagal outflow to the heart rises as you exhale and falls as you inhale, so spending twice as much of each cycle exhaling shifts the balance across the whole breath. What is not established is that 2:1 is better than 1.5:1 or 3:1, or that the ratio matters more than the overall rate does. That gap is why this one is rated Thin Evidence while coherent breathing is not — the rating is about the specific claim, not about slow breathing in general.
It is also the easiest of these to do badly. A forced eight-second exhale makes people tighten the abdomen and then gasp on the way back in, which undoes the point. If that is happening, shorten the exhale until it is comfortable. A relaxed four-and-six beats a strained four-and-eight.

Should I push the air out?
No. Let it fall out. Exhalation at rest is mostly passive — the ribs and diaphragm recoil on their own. If you are squeezing at the end of the count, the count is too long for you today.
Why is the evidence rated thin if long exhales are well established?
Because the rating applies to the claim the page makes, and this page makes a claim about a specific ratio. That the exhale is where vagal activity concentrates is well supported. That 4:8 in particular does something 4:6 does not has never been directly tested.
Can I use it to fall asleep?
It is a reasonable thing to do lying in bed, and plenty of people find it helps. Nothing on this site is a treatment for insomnia, and if you have been sleeping badly for weeks that is worth raising with a doctor rather than pacing your breathing at.
